I am committed to maintaining the trust and confidence of my patients. In particular I want you to know that I’m not in the business of sharing, selling, renting or trading your personal information with other organisations for marketing purposes. I just don’t do that sort of thing. But in case you don’t believe me this privacy policy provides lots of detailed information on what personal information is collected, how it's used, the limited conditions under which it might be disclosed and how it's kept secure. Boring yes, but it’s important to you so that means it’s important to me.
PERSONAL INFORMATION COLLECTED DIRECTLY FROM YOU
Snowdonia Osteo collects personal information when you register as a new patient, if you book an appointment or make an enquiry through the website, or if you book an appointment or get in touch via email, phone or social media.
Information collected may include your name, gender, date of birth, home address, email addresses, telephone numbers and health insurance numbers.
During the new patient registration and ongoing consultation processes you will be asked for information about your current health, past medical history and any medication you are currently taking or have taken in the past.
Snowdonia Osteo does not directly collect any special categories of personal data about you, such as details about your race or ethnicity, religious or philosophical beliefs, sexual orientation, political opinions or criminal convictions.
PERSONAL INFORMATION COLLECTED INDIRECTLY
Like most websites www.snowdoniaosteo.com uses data retrieved from cookies and other technology which gathers information about website use in order to ensure effective operation of the site and to make your interaction with it as smooth as possible.
Information collected may include pages visited, time spent on pages and other anonymous traffic data.
The Snowdonia Osteo newsletter gathers statistics using click data to improve it’s future performance (for more information please see MailChimp’s privacy statement) and can be unsubscribed from at any time by clicking on the ‘unsubscribe’ link.
Cookies do not allow access to your computer or you to be identified as an individual.
If you want to disable cookies you need to change your website browser settings to reject them, but please bear in mind that doing so may affect how well the website functions for you.
HOW INFORMATION IS USED
Generally speaking personal information collected is used for purposes of legitimate interest such as registering you as a patient, appointment confirmations and reminders, retrieving your medical records during a consultation and for private communication with you regarding your ongoing treatment and management.
When you register as a patient you’ll be asked whether you are happy to receive the occasional newsletter containing news, tips, updates and offers. You are perfectly entitled to opt out of these at the time or at any point in the future, either by clicking on the link at the bottom of any such email or by contacting me directly at hugo@snowdoniaosteo.com.
HOW AND WHEN INFORMATION CAN BE DISCLOSED
All information gathered during your initial consultation, subsequent treatments and on-going management is completely confidential
Except as described in this privacy policy your data will never be shared, rented, sold or traded with other organisations without your consent unless it is legally required to do so.
It might be necessary to consult your GP or another specialist at some point but this will only ever be done after first discussing the matter with you and gaining your consent.
In some circumstances it might be necessary to compile a treatment report for your employer but this will only ever be done after first discussing the matter with you and gaining your consent.

HOW TO ACCESS, CORRECT OR DELETE YOUR PERSONAL INFORMATION
You can request access to your personal data by emailing me directly at hugo@snowdoniaosteo.com.
If you think there is a problem with the personal information held you can request a correction, although the accuracy of the new data provided will need to be verified.
If you want to request erasure of your personal information all reasonable steps will be taken to comply unless we are legally required to keep it; patient records are stored for a statutory minimum of 8 years after the last appointment, or up to the patient's 25th birthday if aged under 16 at the time of the first consultation, after which you may request that they are destroyed.
